The plan can then be written demonstrating how the person wants to be cared for and supported and how the resources available to them will help achieve this. The resources would include support from family and friends, for example a friend may be happy to take the person’s shopping or a neighbour might assist them with their garden. The person may also include the connections they have in the community, such as groups, activities and grants that they are accessing.

The person is entitled to a copy of their assessment and their care and support plan. Having access to a copy of the completed assessment will prove useful when completing the care and support plan as reference can be made to how the person’s outcomes and needs, as identified in the assessment, are going to be met.
